Job Summary:
The Community Health Worker will play a crucial role in promoting and improving community health outcomes within a specified geographic area or target population. This position requires a strong understanding of sociology and community health principles to effectively engage, educate, and empower individuals and communities to make positive health-related decisions. The Community Health Worker will collaborate with various stakeholders and provide culturally sensitive support to individuals, families, and groups to address health disparities and promote overall well-being.
Responsibilities:
1. Conduct outreach activities to identify and engage individuals and communities in need of health-related services.
2. Build strong relationships and trust with community members to provide support, guidance, and resources.
3. Assess individuals' health needs, social determinants of health, and barriers to access healthcare services.
4. Develop individualized care plans in collaboration with clients, considering their specific cultural, social, and economic circumstances.
5. Provide health education on topics such as chronic disease management, nutrition, mental health, and preventive care.
6. Facilitate access to appropriate healthcare services, including referrals to medical providers, community agencies, and support groups.
7. Coordinate and participate in community health events, workshops, and support groups to promote health awareness and prevention.
8. Advocate for clients' needs within the healthcare system and assist with navigating complex healthcare systems.
9. Collect and maintain accurate records of client interactions, services provided, and outcomes achieved.
10. Collaborate with interdisciplinary teams, including healthcare professionals, social workers, and community organizations, to enhance the delivery of holistic care.
11. Stay updated with current research, trends, and best practices in community health promotion and social sciences.
Qualifications:
1. Bachelor's degree in Sociology, Social Work, Community Health, or a related field. Master's degree preferred.
2. Proven experience working as a Community Health Worker or in a similar role.
3. Strong knowledge of sociology principles and an understanding of the social determinants of health.
4. Familiarity with community health programs, resources, and health promotion strategies.
5. Excellent communication and interpersonal skills to effectively engage diverse populations.
6. Cultural sensitivity and ability to work with individuals from various ethnic, cultural, and socioeconomic backgrounds.
7. Proficiency in conducting assessments, developing care plans, and implementing health education programs.
8. Ability to maintain confidentiality and demonstrate professionalism in all interactions.
9. Strong organizational skills to manage caseloads, documentation, and follow-up activities.
10. Proficiency in using computer systems, databases, and software relevant to the position.
11. Ability to work independently and as part of a team in a dynamic and fast-paced environment.
12. Valid driver's license and reliable transportation required.
